Breast-Conserving Surgery & Mastectomy

The cancerous lump and a surrounding margin of healthy tissue are removed while the rest of the breast is preserved (lumpectomy/breast-conserving surgery), or the entire breast is removed (mastectomy, including skin- or nipple-sparing techniques) when the tumour is large, multifocal, or genetic risk favours it. Breast-conserving surgery is followed by radiotherapy in most cases and gives equivalent long-term survival to mastectomy for eligible early-stage cancers.

Lumpectomy is usually a day case or one overnight stay, with light activity resuming within days and most normal activity by 2 weeks; mastectomy typically involves a 1–3 day hospital stay with drains left in for a week or more, and full recovery over 4–6 weeks (longer if combined with immediate reconstruction). Watch for wound infection, seroma (fluid collection), bleeding, and — with mastectomy — numbness or reduced shoulder mobility; a second surgery is occasionally needed if margins are not clear.

Sentinel Lymph Node Biopsy & Axillary Clearance

One or a few 'sentinel' lymph nodes — the first nodes cancer would reach — are identified with a tracer dye or radioactive marker and removed to check for spread, avoiding removal of the whole underarm node group when they are clear. If sentinel nodes contain cancer, a fuller axillary (underarm) lymph node dissection may follow to remove more nodes.

Sentinel node biopsy alone is usually done alongside the breast surgery and adds little extra recovery time, with most arm symptoms resolving within 1–2 weeks and a lymphedema (arm swelling) risk of roughly 5–17%. Full axillary dissection, needed only when sentinel nodes are positive, involves a longer recovery (2–4 weeks) and a meaningfully higher lymphedema risk of roughly 20–30%, along with possible numbness, stiffness, or nerve irritation in the arm.

Chemotherapy, Hormonal & Targeted Therapy

Systemic drug therapy tailored to the tumour's biology: chemotherapy for higher-risk or larger tumours, hormonal therapy (such as tamoxifen or an aromatase inhibitor) for hormone-receptor-positive cancers, targeted anti-HER2 therapy (such as trastuzumab) for HER2-positive cancers, and increasingly a CDK4/6 inhibitor added to hormone therapy for certain cancers. This is given before surgery (neoadjuvant, to shrink the tumour) or after (adjuvant, to reduce recurrence risk) over a protocol lasting months.

Chemotherapy is usually given in outpatient cycles every 2–3 weeks over 3–6 months, with fatigue, nausea, hair loss, and a temporary drop in blood counts (raising infection risk) as the main things to manage; hormonal and targeted therapies are typically daily tablets or periodic infusions continued for months to years with milder day-to-day side effects but their own risks (e.g., cardiac monitoring with anti-HER2 therapy). Most patients continue this phase of treatment closer to home once travel-based surgery is complete.

Breast Radiotherapy & IORT

Focused radiation is delivered to the remaining breast tissue and sometimes nearby lymph nodes after breast-conserving surgery (standard practice, to reduce local recurrence), or after mastectomy in higher-risk cases. Newer techniques such as intraoperative radiotherapy (IORT) or partial breast irradiation deliver treatment in fewer sessions for suitable early-stage cases.

Conventional whole-breast radiotherapy is given as daily outpatient sessions over 3–6 weeks (shorter accelerated schedules are increasingly used); it does not require hospital admission but does require the patient to remain locally for the full course. Skin redness and irritation similar to a sunburn, fatigue, and mild breast swelling are common and usually settle within weeks of finishing; rare longer-term effects include changes in breast skin texture or, very rarely, lung or heart tissue effects depending on which side is treated.
